Provider Search Services Directory Terms & Conditions

Provider Search, LLC’s www.providersearch.com Website (“the “Website”) offers families several ways to identify and locate professional care providers for individuals with developmental disabilities, including advertisements for providers, a searchable directory of providers, and third-party provider referral systems.

One of the owners of Provider Search, LLC is also a shareholder of Independent Life Services, Inc. (“ILS”). ILS is a contracted provider for the Department of Economic Security / Division of Developmental Disabilities. ILS’s mission is to promote, encourage and empower individuals with developmental disabilities to live a fulfilling life as independently as possible. ILS has facilities in Mesa, Arizona and Queen Creek, Arizona. Provider Search, LLC provides the same services and exposure for ILS as it does for any other qualified provider who becomes a Member of ProviderSearch.com.

Acceptance

Provider Search reserves the right to reject any proposed provider listing for any reason.  Provider Search’s listings of providers who are contracted with the Arizona Department of Health and Safety will be done in compliance with A.R.S. § 36-557.

References to Provider Search

You agree not to represent your advertisement in the Directory as an endorsement, referral, or recommendation by Provider Search, LLC

Advertising Rates

There is presently no charge for providers to advertise on the Provider Search Web site.   Provider Search reserves the right to change its pricing and billing policies at any time. 

Termination

Provider Search reserves the right to terminate your advertisement at any time. This could occur if: (i) you are disciplined by an Arizona state regulatory agency,  or (ii) Provider Search receives significant complaints from customers. The determination of whether complaints are significant is within Provider Search's sole discretion.

LIMITATION OF LIABILITY.  IN NO EVENT SHALL PROVIDER SEARCH BE LIABLE FOR ANY LOSS OF PROFIT OR ANY OTHER COMMERCIAL DAMAGE, INCLUDING BUT NOT LIMITED TO SPECIAL, INCIDENTAL, CONSEQUENTIAL OR OTHER INDIRECT DAMAGES  UNDER ANY CAUSE OF ACTION ARISING OUT OF OR RELATING TO THIS AGREEMENT, INCLUDING, WITHOUT LIMITATION, CLAIMS ARISING FROM MALFUNCTION OR DEFECTS IN THE DIRECTORY OR THE WEBSITE.  PROVIDER SEARCH'S MAXIMUM LIABILITY HEREUNDER IS EXPRESSLY LIMITED TO THE AMOUNT PAID UNDER THIS AGREEMENT BY THE CONTRACTING PROVIDER TO PROVIDER SEARCH WITHIN THE THREE (3) MONTH PERIOD IMMEDIATELY PRECEDING THE CAUSE GIVING RISE TO THE CLAIM.

Changes to Provider Search’s Directory Terms and Conditions

These terms and conditions may change, and you will be subject to such changes. It is your responsibility to periodically review these terms and conditions.
